Otto the Flesheater is one of the hardest fights Mistfall Hunter throws at you. He hits fast for his size, soaks up punishment from ranged attacks, and punishes disorganized teams hard. Getting to him takes preparation, and the fight itself demands you read his telegraphs correctly or get wiped. This guide covers exactly where to find him, every attack in his kit, and the team composition that makes this fight manageable.
Where do you find Otto the Flesheater in Brandrgarde?
Otto spawns exclusively in Brandrgarde, a zone that requires a minimum equipment value of 300 Gyldenblood before you can enter. Once inside, head to the Main Hall area near the center of the map. The arena doors stay sealed until roughly the 20-minute mark, when Soekja (the White Raven) sends a warning cue on screen signaling that the Mist Lord is ready to be fought.
The Main Hall has three entrances: East, South, and West. The moment your team steps inside and engages, the barrier seals behind you, which means no other squads can interfere mid-fight. That's the good news. The bad news is that the same seal works against you if you're not prepared.

Main Hall arena entrance
After the fight ends, a portal appears. You have roughly 45 seconds to loot Otto and reach it before other teams flood in. The portal drops you at the center of the final Gyldenmist corruption zone, so bring Souls of Return before you ever set foot in this arena. Running out of extraction options after the kill is a painful way to lose your gear.
What are Otto the Flesheater's attack patterns?
Otto's moveset is melee-heavy with a few ranged options mixed in. His attacks split into two categories: blockable moves you can parry and Red Attacks that you cannot. Learning which is which saves your health bar.

Corrupted Ground Smash AoE
Blockable and parriable attacks
- Pounce Attack: Otto prowls, then leaps forward. On landing he slashes to both sides. Dodge forward just before impact. Dodging sideways puts you directly in the follow-up claw swipe.
- Berserk: A forward-moving flurry of claw strikes. Step to his sides or back out of range. Both blocking and parrying work here.
- Ground Smash: Otto pounds the ground and sends an arc of rocks outward. Dodge or stay outside the AoE radius.
Red attacks (cannot be blocked or parried)
- Charged Pounce: Same animation as the standard Pounce but Otto glows red. Do not try to block this. Dodge through it.
- Berserk Fury: Otto screams and charges the aggroed player with a chain of attacks. The only reliable counter is distance. Get out of his path entirely.
- Corrupted Ground Smash: Otto becomes empowered by Gyldenmist corruption and glows gold before smashing the ground. This creates a branching explosion pattern and applies a movement speed debuff on hit. Stay well clear.
- Gyldenblod Puke: When Otto glows yellow and screams, he vomits Gyldenblod across the arena floor, leaving spreading AoE damage pools. If you are holding aggro, pull him toward the northern wall and hold position. Moving around spreads the pools and makes the arena unworkable for your teammates.
In his final phase, golden fire begins erupting at random spots across the arena, shrinking the safe space available. There is a hard timer on this phase: if you do not kill him fast enough, he reaches Enraged mode, which stuns and wipes the entire party.

Mistfall Hunter Boss Guide: How to Beat Otto the Flesheater
What is the best team composition for the Flesheater fight?
After running this fight with multiple setups, the composition below handles Otto most consistently.
The Withered Knight works best in the Polearm and Shield build, holding Otto's attention and blocking every attack that allows it. The Mercenary fills the same frontline role and is a solid alternative if you want a different playstyle. For a guide on maximizing the Mercenary's kit, the best Mercenary build guide breaks down the top sword and shield skills and talent choices.
Your Sorcerer or Blackarrow exists purely to chip down Otto's HP. The Flesheater resists ranged attacks and magic more than melee, so the DPS player needs to stay consistent rather than burst. A Shadowstrix is a viable swap here if you want extra mobility during aggro resets.
The Seer is the glue. Its buff and debuff AoEs keep the fight from spiraling, and its Shield generation gives the tank breathing room during the tougher Red Attack sequences. During the fight, the Seer should rotate between the back of Otto and the frontline to decompress the tank while keeping heals cycling.
Quest prerequisites: The Flesheater's Feast
If you are working through the main story, the Flesheater's Feast quest is the formal objective tied to killing Otto. Before it unlocks, you need to complete four questlines: Iron and Fire, Life Must Go On, Reunion, and Siblings.
Once those are done, upgrade your camp Tavern to level 2 using 2 Norgard King's Coins, 2 Ritual Severed Daggers, 2 Crimson Bat Eyes, and 1 Bone Lyre. Speak to Dew after the upgrade and she assigns the kill.
Post-kill: Don't waste the 45-second window
The moment Otto dies, a portal opens. Other teams have been waiting outside the sealed arena and will rush in as soon as the barrier drops. You have approximately 45 seconds to loot and reach the portal. Assign one player to loot immediately while the rest watch the entrances.
Remember: the portal deposits you at the center of the final Gyldenmist corruption zone. If you did not bring Souls of Return before the fight, you could find yourself unable to extract after one of the game's toughest kills.
