From clothing line to official D&D canon

Death Saves started as Manganiello's own dark fantasy clothing brand, one that had gone quiet for years with stock left unrenewed. The aesthetic was always there: skulls, heavy metal imagery, and a love for D&D's darker corners. Now he's channeling all of that into a full-blown official setting for the tabletop RPG.

The world Manganiello has built centers on the aftermath of the Blood War, the eternal conflict between the devils of Baator and the demons of the Abyss. In Death Saves, that war is over. The gods are dead. The world is barely holding together. Here's the thing: that premise is a genuinely different creative direction for D&D, which has historically kept its cosmology intact and its deities very much alive.

The setting was co-developed with Mike Mearls, co-lead designer on D&D's 5th edition and former franchise creative director at Wizards of the Coast. Mearls was laid off from Wizards in 2023, making Death Saves his first direct involvement with a mainline D&D product since then. That collaboration carries weight. What the Black Label line actually means

The Black Label branding is doing real work here. DC Comics runs a Black Label imprint for mature, adult-targeted stories that sit outside the main continuity. Manganiello described his approach as "drawing from D&D's past to chart its future," which tracks with the Blood War being a long-standing piece of D&D lore that has rarely been given a full setting treatment.

Where this fits for D&D players right now

The D&D universe is expanding in multiple directions simultaneously. The Forgotten Realms remains the default, Greyhawk recently got renewed attention through organized play, and now Black Label is carving out space for something heavier. Players who have felt that mainstream D&D products play it too safe finally have an official home for that grimdark instinct.
